Definition
Freight sector scorecard
Any incident involving a:
- Brake defect which can include the following;
- Sparks coming from a train
- Brakes isolated
- Air leaks
- Dragging Brake
- Fire – leading from a vehicle component failure
- Smoke coming from train
- Defective handbrake
- Runaway of vehicle (competent failure, not person failure. If person failure use Railway Operating Incident)
- Wheel defect not picked under the WILD definition
- Hot axle box detection (HABD)
- Train division (competent failure, not person failure. If person failure use Railway Operating Incident)
SMIS sub-event/s
- Train axle wheel loading fault or detection (sub-event guidance)
- Train fault (sub-event guidance)

Notes
- Only brake isolations that occur whilst the train is operating on the mainline (i.e. an in-service wagon defect) need to be entered into SMIS. If it occurs prior to going onto the mainline, the event does not need to be entered into SMIS?
