Freight sector scorecard

Any incident involving a:

  • Brake defect which can include the following;
    • Sparks coming from a train
    • Brakes isolated
    • Air leaks
    • Dragging Brake
    • Fire – leading from a vehicle component failure
    • Smoke coming from train
    • Defective handbrake
    • Runaway of vehicle (competent failure, not person failure. If person failure use Railway Operating Incident) 
  • Wheel defect not picked under the WILD definition 
  • Hot axle box detection (HABD)
  • Train division (competent failure, not person failure. If person failure use Railway Operating Incident)

Notes


  • Only brake isolations that occur whilst the train is operating on the mainline (i.e. an in-service wagon defect) need to be entered into SMIS. If it occurs prior to going onto the mainline, the event does not need to be entered into SMIS?
